সন্মান সহকাৰে আপোনাক জনাব খোজোঁ যে মই ২০০১ চনতে এক লাখ টকাৰ এটা জীৱন বীমাৰ পলিচী খুলিছিলো। তেতিয়া মই ‘নমিনী’ হিচাবে মোৰ মাৰ নামটো দিছিলোঁ। কিন্তু ইতিমধ্যে ২০০২ ত মোৰ মাতৃ বিয়োগ ঘটে। গতিকে মই এই আবেদনৰ জৰিয়তে মোৰ উক্ত পলিচীখনত ‘নমিনী’ হিচাবে মোৰ মাতৃৰ ঠাইত মোৰ জীয়ৰী ৰূপমতীৰ নামটো সন্নিবিষ্ট কৰিবলৈ অনুৰোধ কৰিলোঁ। মোৰ অবিহনে উক্ত পলিচীখনৰ সমুদায় ধনৰাশি মোৰ জীয়ৰী ৰূপমতীয়ে পাব। আশাকৰোঁ বিহিত ব্যৱস্থা কৰি পলিচীখনৰ নমিনীৰ নামটো সোনকালে বদলি কৰি দি বাধিত কৰিব।

Sir,
With due respect I would like to state that I opened a LIC Policy with one lakh sum assured money. That time I had nominated my mother’s name in the policy. But my mother had expired in the year 2002. So I would like to change the name of my nominee from my mother’s name to my daughter’s name Roopmati in the said policy.
Therefore, I request you to kindly make necessary arrangements to change the name of the nominee forthright in the said policy and I declare that henceforth my daughter Roopmati will be my sole nominee regarding the policy after my death.
