মাননীয় মহোদয়,
অধীনৰ বিনীত নিবেদন এই যে মই নগাৱতে আজি দহ বছৰ ধৰি বাস কৰি আহিছোঁ। বৰ্তমান মই ভাৰাঘৰতে আছোঁ। মোৰ চাকৰিৰ কাৰ্য্যকালো আৰু বেছিদিন নাই। সেয়েহে চাকৰি থাকোতেই ইয়াতে স্থায়ীভাৱে থাকিবলৈ নিজাকৈ এটা ঘৰ সাজিবলৈ স্থিৰ কৰিছোঁ। সেই উদ্দেশ্য ইয়াৰে আমোলাপট্টীত দুইকঠাৰ এটুকুৰা মাটি কিনিছো। ইতিমধ্যে মই ঘৰৰ যাৱতীয় নক্সা আদি তৈয়াৰ কৰিছো। মাটিৰ খাজনা দিয়া ৰচিদসহ-ঘৰৰ নক্সা আদি এই আবেদন পত্ৰৰ লগত গাঠি দিছো আপোনাৰ বিবেচনাৰ্থে।
গতিকে দয়ালু মহোদয়ৰ ওচৰত মোৰ সৰল মিনতি যেন ঘৰ সজোৱাৰ যাৱতীয় অনুমতি সোনকালে দি মোক বাধিত কৰে যেন।
ইয়াৰ লগতে মাটিৰ খাজনা দিয়া ৰচিদৰ ফটোকপি আৰু দুখন ঘৰৰ নক্সা গাঠি দিয়া হ’ল।

Respected Sir,
With due respect I would like to state that I have been living in Nagaon since last ten years in the rented house. My service period is also coming to end soon. Therefore, I have decided to live here permanently for which I would like to construct a house of my own. I have already purchased a plot of land measuring “two Katha” at Amolapatty here. I have also prepared necessary plans and drawings of the proposed house. As such land-revenue receipt and the plan and drawing of the house has been attached with this application for your kind consideration.
Therefore, Sir, this is my humble request and prayer to you to accord necessary permission to me soon to construct the proposed house and oblige.

1. A Xerox copy of land revenue receipt.
2. Two copies of plan and drawing of the proposed house.
