মাননীয় মহাশয়,
অধীনৰ বিনীত নিবেদন এই যে দিশপুৰ বিধান সভাৰ সমষ্টিৰ অন্তৰ্গত আৰু কামৰূপ মহানগৰ জিলাৰ ভিতৰৰ উলনী গাঁৱৰ মই এজন নাগৰিক। এই বছৰ (২০১১) মই এজন ভোটাৰ হৈছো আৰু ভোটাৰ লিষ্টত মোৰ নাম প্ৰকাশিত হৈছে।
কিন্তু মহাশয় অতি দু:খেৰে আপোনাক জনাব লগীয়া হৈছোঁ যে যিদিনা ভোটাৰসকলৰ পৰিচয় পত্ৰত ফটো তোলা হৈছিল সেইদিনা মই গাৱতেই নাছিলোঁ যাৰ বাবে মোৰ ফটো তুলিব নোৱাৰিলে।
এতিয়া মোৰ চিন্তা হৈছে ভোটাৰৰ ফটো পৰিচয়-পত্ৰ নোহোৱাকৈ মই আগন্তুক নিৰ্বাচনত ভোট দিব পাৰো নে নোৱাৰো। সেয়েহে দয়ালু মহোদয়ৰ ওচৰত মোৰ বিনম্ৰ অনুৰোধ যেন মোৰ ভোটাৰৰ পৰিচয়-পত্ৰত ফটো তুলিবৰ বাবে বিহিত ব্যৱস্থা কৰি মোৰ ভোটাধিকাৰ সাব্যস্ত কৰিবলৈ সুবিধা দিয়ে।

Respected Sir,
I beg to state that I am an inhabitant living in Uloni Village within the Dispur LAC in the Kamrup (metro) district. This year (2011) I have become eligible as a voter after attaining 18 years of my age. My name as a voter has also appeared in the draft voter’s list.
But Sir, with great sorrow I would like to inform you that I was absent in the village on the date photographs were taken on the voter’s photo identity card. As a result I missed the photo session.
Now I am quite perturbed as to whether I would be able to cast my vote in the coming election without possessing my photo identity card.
Therefore, Sir, this is my humble request to you that arrangement may be made to take my photograph in the voter’s identity card so as to enable me to exercise my franchise.
