মহাশয়,

অধীনৰ বিনীত নিবেদন এই যে মোৰ স্বামী শ্ৰীনন্দ দুলাল উপাধ্যায়ৰ যোৱা ২০ নবেম্বৰত মটৰ দুৰ্গটনাত মৃত্যু হয়। মৃত্যুৰ সময়ত তেওঁ ৪৮০০০৫৬৭৮ নম্বৰৰ জীৱন বীমা পলিচীখন এৰি গৈছে। উক্ত পলিচিখনৰ ধনৰাশি সমুদায় মোক আদায় দিবলৈ এই পত্ৰৰ দ্বাৰা আবেদন জনালোঁ যিহেতু মই তেওঁৰ স্ত্ৰী হোৱাৰ উপৰিও উক্ত পলিচীখনৰো মই ‘নমিনী’।

I beg to state that my husband Shri Nanda Dulal Upadhyaya expired on 20.11.12 due to motor accident. At the time of death my husband left a LIC policy bearing No. 480005678.

Therefore, as per rule I request you kindly to pay the entire insured amount to me at an early date and oblige. I am his wife as well as the nominee of the said policy.
