Sir,
With due honour I beg to inform you that I have lost my UBI-VISA credit card issued by your branch on 20.05.2006.(No. NB 253041), while coming back from the office. Please immediately stop all transactions on this card and issue me a new card along with a new pin mailer and at the same time cancel my existing card. Copy of the G.D. is attached herewith.
Hope you will do the needful at the earliest.

যথাযথ সন্মান সহকাৰে জনাব বিচাৰো যে আপোনাৰ বেঙ্কৰ পৰা যোৱা ২০/ ০৫/ ২০০৬ তাৰিখে মোৰ নামত ইছু কৰা SBI VISA ক্ৰেডিট কাৰ্ড খন (নং NB 253041) অফিচৰ পৰা অহাৰ পথত হেৰাই গৈছে। আপোনাক অনুৰোধ জনাওঁ যে অনুগ্ৰহ কৰি এতিয়াই এই কাৰ্ডৰ সকলো প্ৰকাৰৰ কাম-কাজ বন্ধ কৰি দিয়ক আৰু মোক এটা নতুন পিন-মেইলাৰ সহ এখন নতুন কাৰ্ড ইছু কৰক, লগতে বৰ্তমানৰ ক্ৰেডিট কাৰ্ড খন বাতিল কৰি দিয়ক। ইয়াৰ লগতে মই জি.ডি.ৰ. এটা কপি গাঠি দিলো। আশা কৰো আপুনি অতি শীঘ্ৰে প্ৰয়োজনীয় ব্যৱস্থা ল’ব।
