মহাশয়,
সন্মানপূৰ্বক নিবেদন এই যে আপোনাৰ বেঙ্কত মোৰ ১২৩৪ নং এটা চেভিংছ একাউণ্ট আছে আৰু যোৱা দহ বছৰ ধৰি সেই একাউণ্টৰপৰা টকাৰ লেনদেন কৰি আহিছোঁ। যোৱা সপ্তাহত মই সেই পাছ-বুকখন জমা-খৰচৰ হিচাব তুলিবলৈ বেঙ্কত জমা দিছিলোঁ। কিন্তু পিচত ঘূৰাই আনিবলৈ যাওঁতে পাছ-বুকুখন নাই বুলি ক’লে। বহুত বিচাৰিও পোৱা নগ’ল।
গতিকে আপোনাক অনুৰোধ জনাওঁ যাতে ইয়াৰ ব্যৱস্থা কৰি মোক এখন পাছ-বুকৰ যোগান ধৰে।

Sir,
With due respect I would like to state that I have a Savings Bank Account No. 1234 in your Bank from which I have been making transactions since last ten years. Last week I deposited the pass-book to update the transactions. But, when I went to receive the same the clerk told me that he could not find it. On searching thoroughly the pass-book could not be traced.
Therefore, I request you to kindly do the needful and give me the necessary pass-book.
