I beg most respectfully to state that I have been laid up with fever since last week. My attending physician has diagnosed it to be a case of malaria, and advised me for rest of one week.

So, therefore, I pray to you please grant me 2 weeks of sick leave from 1.2.98. with this application I do give a medical certificate also for your records.

বিনীত নিবেদন এই যে, মই যোৱা সপ্তাহৰ পৰা জ্বৰত শয্যাশায়ী হৈ আছো। মোৰ চিকিৎসকে ইয়াক মেলেৰিয়া বুলি চিনাক্ত কৰি চিকিৎসা কৰি আছে আৰু এসপ্তাহ বিশ্ৰাম ল’বলৈ কৈছে।

সেয়েহে মই প্ৰাৰ্থনা কৰো যে যোৱা ১ ফেব্ৰুৱাৰী ১৯৯৮ পৰা দুসপ্তাহৰ বাবে অসুস্থতাৰ ছুটী মঞ্জুৰ কৰি বাধিত কৰিব। এই আবেদনৰ লগতে ডাক্তৰৰ প্ৰমাণপত্ৰ এখনো গাঠি দিলো।
